Actions > Injury > Report an injury.

From here complete the required fields marked with an asterisk.  This is the minimum required to record an injury.

Once submitted, depending on if you want to add more details, you can at any time.  Additionally more information can be added to fulfil the needs of an HSE Riddor submission.
